I'd use these fields:

Name, Email, Phone (optional), Portrait Experience, Preferred Date or Timeframe, and Tell Me About Your Vision.

For Portrait Experience, use a dropdown with Signature Portraits, Boudoir + Sensual, Fantasy + Vintage, and Not Sure Yet.

Include a privacy notice and a link to your Privacy Policy.

In Wix, you can connect the form to email notifications so inquiries reach you directly.

Two things to confirm before publishing

1. Are you currently accepting portrait bookings? If not, we should change the consultation section to a waitlist rather than suggesting sessions are available.

2. What exactly is included in the $395, $495, and $595 starting prices? We should specify session duration, number of edited images, whether digital files are included, and any deposit requirements. Otherwise, potential clients may hesitate because they don't know what they're purchasing.

My recommendation is to build this as one polished studio page first, using the photography and materials you already have. Once it's live and functioning, we can develop individual experience pages as needed.
